Element Concepts:

First, due to the dynamic nature of the form and the processing power needed to have smooth transitions for the form, I disabled the animation on the flag.

While there is an option to select complaints, once it has been selected in the final version the content scroll will roll up and be animated onto a plank where it jumps or is taken by a kraken down to Davey Jones locker. The form will then unroll again and the complaint option will be disabled. I understand that this will probably lead to complaints being directed to another option on the form, but if too many come in under another category then I may re-enable to option to submit a complaint. I tried to comment the code in the JS well enough that you will be able to see where certain options will be directed.
